MEMBERSHIP SETUP & REQUIREMENTS
Member and Pet Profile
In order for us to care for your dog in a safe and responsible manner, and to ensure that he or she has as much fun as possible, we require that each parent fill out a detailed profile. A complete profile is required for admittance and includes important information such as an authorized persons on your account, your veterinarian’s name, dates of latest vaccinations, and of course your dog’s birthday. No dog will be cared for unless all information is in our system is accurately represented.
Each member must thoroughly read and sign a Member Service Agreement that releases PUPS from certain liabilities. By utilizing our services and signing the agreement, you understand and acknowledge that PUPS has limited liability regarding certain occurrences and incidents that may occur while your dog is in our care. While we take every precaution possible to ensure that all of the dogs in our care are healthy and safe, you must recognize that incidents will happen whenever dogs socialize together that are beyond our control. We cannot take responsibility for your dog’s behavior or the behavior of other dogs in our care. Therefore, PUPS is not liable for injuries or illness that any animal may incur while in our care. PUPS also reserves the right to quarantine, transport and/or take reasonable action regarding any animal in our care to insure their well-being and the well-being of other dogs in our care. In addition the release of liability every member must agree to accept that PUPS does take video or photographs of members, that members have no rights to these videos or photographs, and PUPS may use them without limitation or compensation to the member. No dog will be admitted into our care unless this agreement is fully understood, executed and verified in our system. In order to use our services, you must have a valid credit card on file with us at all times. Each dog must have a local veterinarian that is authorized to see and treat your dog if medically necessary. While we take every precaution possible to safeguard your dog’s health, things can and do happen on occasion. If you do not have a local veterinarian, we would be happy to recommend one to you. PUPS abides by all local, state, and federal laws and regulations that require us to insure that dogs that we care for are fully vaccinated. In most states these requirements include Rabies, Distemper/Parvo, and Bordatella. PUPS reserves the right to require additional vaccinations for specific services as recommended by local medical professionals to help insure the health and safety of every dog we care for. Every PUPS member, no matter what club you visit, must comply with the state and local laws and regulations by providing PUPS with a current health certificate verifying that all required vaccinations are up to date. All dogs using our services must be healthy as we care for a large number of animals. We will not admit any dog that we believe to be a risk to the other animals in our care. It is your responsibility as a parent to notify us of any health related issues affecting your dog so that we can make an informed determination about how to best care for them. Members who do not inform us in a timely manner of any health issues, knowingly bring their dog to us while he or she is sick, or do not make all efforts possible to remove their dog from our facility once determined to be a risk by us may not be allowed to use our services again.
